Provider Instructions & Information


If you're here because you've been asked to be a Provider for a specific University of Idaho event, please enter the date of the event you'll be a Provider for - if it's a multiple-date event, enter the LAST date.
(entering the last date will insure that no licenses or certificates, etc., will expire before your event's final date)

Note - ALL of these expiration dates must be AFTER the last date of your event!

If you've never been a Provider for a University of Idaho event where alcohol is served, or if one or more of your dates have expired (and show in RED) you'll find the steps you need to take to be a Provider for an event below.

If you're a new Provider, you've probably noticed above that your Certificate(s) of Insurance, Agreement(s), and - if you're an Alcohol Provider, your Liquor License - haven't been uploaded yet, so they show as "01/01/1900",
in RED.

The University of Idaho works with three types of Providers - Alcohol Providers, Food Providers, and Combined (Food & Alcohol) Providers. Depending upon which category you're in, please follow the instructions below:
Alcohol Providers
Certificate of Insurance
  1. You'll also need to print a Request for Certificate of Insurance to give to your insurance agent or broker (print by using the selection from the main Providers page).
  2. When they've given you the required Certificate of Insurance, you'll scan and email the Certificate as a .pdf document to alcohol-admin@uidaho.edu.
Liquor License
  1. Finally, you'll need to email a scanned copy of your Liquor License as a .pdf document to alcohol-admin@uidaho.edu.
Alcohol Provider Agreement
  1. You'll need to print and sign your Alcohol Provider Agreement, which is a selection on the main Providers page. Be sure to fill in your organization's DBA NAME and FULL LEGAL NAME on the webpage before printing your Alcohol Provider Agreement.
  2. Once your Agreement is printed and signed, you'll need to scan and email your Alcohol Provider Agreement as a .pdf document to alcohol-admin@uidaho.edu.
Food Providers
Certificate of Insurance
  1. You'll also need to print a Request for Certificate of Insurance to give to your insurance agent or broker (print by using the selection from the main Providers page).
  2. When they've given you the required Certificate of Insurance, you'll scan and email the Certificate as a .pdf document to alcohol-admin@uidaho.edu.
Food Provider Agreement
  1. You'll need to print and sign your Food Provider Agreement, which is a selection on the main Providers page. Be sure to fill in your organization's DBA NAME and FULL LEGAL NAME on the webpage before printing your Food Provider Agreement.
  2. Once your Agreement is printed and signed, you'll need to scan and email your Food Provider Agreement as a .pdf document to alcohol-admin@uidaho.edu.
Combined Providers (Food & Alcohol)
Certificate of Insurance
  1. You'll also need to print a Request for Certificate of Insurance to give to your insurance agent or broker (print by using the selection from the main Providers page).
  2. When they've given you the required Certificate of Insurance, you'll scan and email the Certificate as a .pdf document to alcohol-admin@uidaho.edu.
Liquor License
  1. Finally, you'll need to email a copy of your Liquor License as a .pdf document to alcohol-admin@uidaho.edu.
Combined Provider Agreement
  1. You'll need to print and sign your Combined Provider Agreement, which is a selection on the main Providers page. Be sure to fill in your organization's DBA NAME and FULL LEGAL NAME on the webpage before printing your Combined Provider Agreement.
  2. Once your Agreement is printed and signed, you'll need to scan and email your Combined Provider Agreement as a .pdf document to alcohol-admin@uidaho.edu.
ALL Providers
  1. Once your documents have been uploaded to this website by Admin, they'll need to be approved by Risk Management at the University of Idaho. You will be notified by email when this is done.
